SCANNED COPY OF FOLLOWING DOCUMENTS ALONG WITH DULY FILLED-IN APPLICATION FORM REQUIRED FOR ADMISSION

Please Send Required Documents through Email :

1)     Recent passport size photograph (not less than 3 weeks).

2)     Full signature of the Candidate on White Paper in Black Ink Pen.

3)     HSCE or equivalent pass certificate and mark sheet issued by the Board.

4)     CHSE or equivalent pass certificate and mark sheet issued by the Council.

5)     Leaving Certificate of last institution attended.

6)     Domicile/ Residential / Nativity Certificate from competent authority (The validity of the resident certificate 

         shall be 5 years as on last date of submission of application form from the date of issue of the said Certificate).

7)     Caste certificate in respect of reserved category.

8)     PWD Certificate in case of persons with disability candidates.

9)     Service Certificate in case of Ex-servicemen/Serviceman.

10)     Green card certificate for Green Card Holders.

11)  Valid Aadhar Card

12)  Valid E-mail Id.

13)  Migration Certificate, Admit Card of Board & Council in case of outside Odisha candidates

14)  Physical Fitness Certificate / Medical Fitness Certificate.

15)  No Objection Certificate from the employer (only for P.B.B.Sc. & M.Sc. Nursing Course)
